A CCM (Client Care Manager) will come to your house and meet you or your loved one to determine how much care is needed.

2. PLAN OF CARE

The CCM will explain the suggested care package to you or your loved one and change it based on feedback to make sure everyone is on the same page.

3. STAFFING

The CCM and the Staffing Officer will discuss who the best caregiver is for you or your loved one based on the care needs.

4. CAREGIVER MATCHING

The CCM and Staffing Manager will tell you who the best caregiver(s) are for you or your loved one.


5. START OF SERVICE

At this point, the CCM will go over the Service Agreement with the care recipient, family, and/or conservator. Rates will vary depending on the care package.

6. CASE MANAGEMENT

A CCM will manage your case and make sure that the care you or your loved ones receive is consistent with the best practices of 1Heart.

7. PERIODIC EVALUATION

Your CCM will regularly visit you to make sure things are going well, so you don’t have to worry about shifts over time.
